Resposta correta: B

Em I, “have gone entirely smoke-free” usa entirely como advérbio de totalidade, logo completely; slightly e generally mudariam radicalmente a informação. Em II, “the smoke from a burning cigarette” indica origem, ou seja, derived from — e o próprio texto mostra por que exhaled by não serve, pois essa é a outra metade da definição: “and the smoke exhaled by the smoker”. Em IV, “approximately 3,000 American nonsmokers die each year” pede nearly ou around.
